The Intake Interview and You

THE KEY TO UNLOCKING A DYSLEXIC'S POTENTIAL IS EARLY DIAGNOSIS AND THERAPY!

We usually conduct an "intake interview" by phone as our initial approach to understanding how your needs can be met by our program. The intake interview may be conducted by our Center Director or one of our state-certified teachers.

WHAT WE WOULD LIKE TO KNOW: (all information is kept in strict confidence)
  1. The name of the person with the problem.
  2. Your relationship to that person (i.e., mother, grandmother etc).
  3. Where does he or she live?
  4. What grade is he/she in and what school is being attended?
  5. What subjects are the most difficult?
  6. Are there relatives that have reading and/or spelling problems?
  7. Has there been any special testing either at school or privately?
  8. Are there attentional problems?
  9. Are there special situations that might be contributing to learning problems such as loss of a parent, change of schools, english as a second language.

Following the intake interview you may decide to have the comprehensive test battery. If so, we will mail or email to you a more detailed questionnaire for you to bring to us at the time of the test. It would be helpful if you can bring the results of other testing that may help us in the dyslexia evaluation. Please include the best method of contacting you during the day and the evening.

DYSLEXIA FACTS

  • 50% of those that are functionally illiterate are dyslexic!
  • Dyslexia is a coding problem:
  • Decoding involves either "sight recognition" or "sounding out" words.
  • Encoding involves the ability to spell words by recalling or sounding out
WHAT A FEW OF OUR PARENTS SAY ABOUT OUR PROGRAM (names available upon request)

DCT has given Clark the greatest gift of all- understanding dyslexia...In 10 months of therapy he is reading everything. His self-esteem is high and his desire to learn is tremendous. Thank You
"Debbie"

You have given me a different child in 15 months. Emily excels in class and can complete her tasks in a timely manner. DCT gave her the tools to be successful.
"Bev"

I cannot accurately express how deeply and significantly this program has affected the entire family. (It) has affected Xander's entire life and his future for the better. Thank you for all your work to develop a program to treat dyslexia so successfully.
"Alice"

(12/27/2005)" My husband and I can't thank you enough for the dedication and heart-felt interest you have shown our son Nicholas...He went from a C, D and F grades to A, B and an occasional C...The method of teaching that The Dyslexia Center has in place is fabulous"
"Jill"

(4/27/2007) " I just wanted to write and tell you how my feelings have changed toward your center. At first I'll be honest as a school employee and being accountable for my very tight budget I was a little concerned about spending as much as it was going to cost for one child to send to your organization."
"Now, I'm here to tell you if I EVER have another child with problems with dyslexia and I feel they need the help you guys have to offer I myself will make the referral."
"You have helped our child tremendously and for any school that may question "Is it worth it?" please tell them to call me at my office. I can see so much improvement in my child (I say ours because all the children I serve is partly mine) that it's UNREAL."
"Thank you so much for bringing her to the reader and all around good student that I know was there and without your help I'm not sure we would have found."
"THANKS AGAIN!!"
Ava
"Feel free to use this in anyway you like to encourage schools to participate with your organization."
